The aim of the study was to analyze the long-term outcomes of proximal closed wedge osteotomy for the correction of hallux valgus. Osteosynthesis was performed using a mini L-plate. The proximal closed wedge osteotomy was retrospectively analyzed on average 52 months postoperatively. A total of 66 patients (86 feet) participated in the study. The AOFAS score was used to evaluate the postoperative outcome. Bone development was assessed based on pre- and postoperative X-rays. 95.5% of the patients were satisfied with the outcome of the surgery, while 3 patients (4.5%) were not satisfied. The HV angle improved on average by 22.1° (from 36.4° preoperatively to 14.3° postoperatively), and the IM angle was reduced from an average of 17.6° to an average of 6.5°. The average shortening of the first metatarsal was 3.2 mm. The average AOFAS score was 78 points. Conclusion: The results confirm that closed wedge base osteotomy is indicated for moderate to severe hallux valgus. The risk of transfer metatarsalgia can be reduced through adequate osteosynthesis.
