Do I understand correctly that I need to have an HDMI signal to use the device? So if I use an internal app such as YouTube, which does not send an HDMI signal, the device is useless?

Correct. The Sync Box can only be used with an HDMI cable. I have it from the PS5 to the Sync Box and from there to the TV. Bluetooth or other wireless connections are not possible.

Yes and no, apparently there are Samsung TVs that have an app that allows synchronisation without HDMI. So directly from TV to box

You don't need the Sync Box to make your lights react with YouTube or Spotify. All you need is the U application and sync. However, if you use YouTube with your box or PlayStation, the sync box is useful.
