Almost 20 years after the publication of "To Err Is Human," the Action Alliance for Patient Safety e.V. (APS) presents a fundamental analysis of the situation and specific demands for improving patient safety with the "White Paper." The author, Prof. Dr. Matthias Schrappe, was the founding chairman of the APS and has not only worked on the theoretical foundations, the survey methodology, the data on frequency, and the economic implications, but has also developed an innovative concept that can serve as a basis for further practical development and health policy assessment of the issue. The central message is: More patient safety is achievable if the right methods for improvement are applied. The APS explicitly advocates for an updated agenda for patient safety in Germany.
