Product details

Case management, as a method of social work, has been used for many years in elder care to support individuals with care needs and their relatives. This methodological approach aims to develop, integrate, and coordinate needs-based care and support settings in collaboration with those affected. This is intended, among other things, to uphold the principle of outpatient care over inpatient care, thus aligning with the widespread desire of those in need of care to be supported in their own homes. Caregiving relatives are also considered, as they can be supported in their caregiving work through relief offers. In Germany, there was no nationwide advisory service with implemented case management until 2009. However, there are regional projects aimed at implementing this approach.
