It is a scientific fact today that routine analysis of semen is an important predictor of male fertility, leading to an increasing demand for other functional tests to assess male infertility. The main objective of the current study is to consolidate scientific and practical efforts to evaluate male infertility, particularly from immunological and genetic perspectives. The current study concluded that defects in the chromatin status of sperm (chromatin decondensation and DNA damage) and the immunological disorders caused by antisperm antibodies (ASA) in serum and seminal plasma can contribute, at least in part, to the etiology of infertility in the studied patients, even in those with normal semen parameters. However, it appears that ASA affects fertility in a different way than sperm chromatin disorders. It was recommended that both sperm chromatin staining techniques and ASA tests could be routinely used as complementary tests for the diagnosis of infertility.
