After a brief theoretical introduction to the development and implementation status of case management in the international context, current implementation examples from social and healthcare institutions in Germany and Austria are presented. Fundamental challenges and opportunities in transferring the theoretical method to the respective organization- and context-relevant conditions in practice are described. In the form of case studies, the implementation of case management in a hospital, in Styrian child and youth welfare, and in an outpatient nursing service with a focus on integration assistance in the form of supported individual living are explained in detail. Each of these three reports includes information on the organization-specific reasons and goals associated with the introduction of case management.
