This thesis presents a concept for the implementation of the expert standard for discharge management by the DNQP. The first research question addresses the actual development of the concept, while the second research question examines the extent to which the problem-solving process can be applied to the implementation of the expert standard. The introduction outlines various factors that affect the care of patients across sectoral boundaries. Hospital discharge and the resulting post-hospital care issues form a highly relevant topic that is now being discussed under the term interface problems.
