Freudian hysteria is widely considered obsolete today, and the famous case studies of the hysterics of that time are often interpreted as expressions of a period-specific overexcitement. However, using the example of dissociative seizures, the author demonstrates that the former hysterical symptoms—currently referred to as conversion and dissociative disorders—continue to occur and present a significant clinical challenge. The involved disciplines of neurology, psychiatry, and psychotherapy struggle with diagnosis and treatment. The author provides a historically grounded yet highly practical overview of the important theoretical developments from psychoanalysis and psychosomatics, as well as current research practices. The prognosis for dissociative seizures is relatively poor, with a high rate of social medical complications. An empirical, prospectively conducted study on predictive factors for the course of the disorder shows that psychodynamic and personality psychological characteristics have significant explanatory value and are relevant for treatment planning.
